AstraZeneca Korea (hereinafter referred to as the "Company") collects the following personal information from you: [Name, Affiliation (Hospital/Institution), Department, Mobile Phone Number, and Email Address] The above information is collected and used for the purpose of conducting the "Scientific Dialogues on aHUS" (hereinafter referred to as the "Event") and related activities organized by the Company. The Company will retain and use such collected personal information of yours for a period of 30 days from the end of the Event.

You may refuse the collection and use of your personal information as described above. However, please note that if you do not agree to the collection and use of this personal information, you may experience difficulties in attending the Event. In addition, we may share your personal information listed above internally to facilitate the event, and we will retain and use your personal information for the purpose of facilitating the event for 30 days after the event ends.
